The Street Division's website was created to assist citizens in obtaining information about Street Division repairs and services. Through the Service Request page you will be able to create and review the status of your service request online. The Tour & Online Help offers an overview of how to create service requests for Street Division using the map and text method. It also offers help on how to review the status of previously submitted service requests.

Creating Service Request
To create your service request you will be asked to "choose" your method of entry. Your options are the Map method or the Text method. Both methods require the customer information to be completed at the end.
Text Method
Text Method allows users to complete their service request by answering questions regarding the problem. Accuracy is important in regard to location and problem type.
Location Information
Address or Intersection: You can complete the location information with an exact address/hundred block and street name or you may use street intersections to complete the location information.
Location Description: Use a brief description to give more details on the location.
Example: by the west curb, in front of stop sign, middle of intersection.
Problem Information
Type of Problem: Choose the type of problem you are reporting from the drop down menu. Note: additional issues require separate requests.
Describe Problem: You will have 255 characters to explain your request. Please be as precise as possible in your description. Helpful information would include where the problem is noticed and when.
Example: when it rains the storm drain backs up, the tree roots are lifting the sidewalk, an accident yesterday knocked over the stop sign, street light on all day.
Once you have filled in all the information, click on Continue to proceed to the Customer Information screen.
Map Method
Step 1 - Problem vicinity
Select one of the following Options:
- Using the Address or Intersection option allows you to search for a specific address or intersection.
- Using the Community option will give you a list of community areas to choose from and allow you to zoom in on the selected community to find your specific location.
- Using the Zip code option will give you a list of Zip codes to choose from and allow you to zoom in on the selected Zip code area to find your specific location.
Once you have completed the option you want, proceed to step 2.
Step 2 - What is the problem related to ?
What is the Problem related to?: Choose a problem type from the drop down list.
(Additional problem types require a separate request)
Step 3 - Retrieve Map
Get Map: Click on the "Get Map" button and the mapping program will take you to the location or area you have pre-selected. Use the tool buttons on the left side of the page to "Zoom In", "Zoom Out", "Pan" to better view the street or problem item.
Step 4 - Select Item
Identify: The symbol displayed would represents the problem item that you selected. Click on the "Identify" button to choose your problem item (For eg. Street Section, Intersection, Traffic Signal, Storm Drains or Street Lights). Once the item is selected on the map, it will be highlighted in yellow.
Note: If you do not see the feature that the problem relates to, click on the "Select Street" button and select the street section nearest to the problem. Add specific location information in the 'Location Description' field once you have clicked on the "Continue" button. This will take you to the final page for submission. Step 5 - Finalize Selection
Continue: The "Continue" button is displayed after the problem location is selected using either the "Identify" button or the "Select Street" button. Click on the "Continue" button if you are statisfied with your selection. You can add any additional comments that you feel are important to help Street Division resolve your issue in the following page.
Additional Information
Location Description: Be as accurate as possible. Include House number and Street Name. If the problem doesn't have a specific address then give an accurate description of the location with reference to landmarks or other information which will help Street Division locate the problem.
Describe Problem: This field will allow you to provide information concerning the discrepancy you are reporting.
Once you have filled in all the information, click on "Continue" to proceed to the Customer Information screen.
Map Method - Quick Tips
Simple procedure to create a Service Request using the Map Method
- Fill in the Address or Intersection / Select Community or Zipcode. If you are putting in an Intersection, use an "&" sign between the two street names.
(Eg: Novara & Devonshire). For Address searches a street number is mandatory. (Eg: 2781 Caminito Chollas)
- Select the Problem that you are reporting.
FYI: Based on the problem selection either street sections, intersections, storm drains, traffic signals or street lights are made visible for selection on the map.
- Click on the "Get Map" button.
Note: Unless you go through the above three steps sequentially, you won't be able to select the problem location using the "Identify" or "Select Street" tools.
- Once the map loads with your selection parameters, you can use the "Zoom In/Out" or "Pan" tools to better view your problem location.
- Use the "Identify" tool to identify the problem location.
Note: If a particular Storm Drain, Street Light or Traffic Signal is not visible on the map, please use the "Select Street" tool and identify the closest street section.
- Once your problem location is highlighted in yellow, click on the "Continue" button if you are satisfied with your selection and proceed to fill in the remaining fields to create a service request.
Customer Information
Customer Information: In order to provide you with the best possible customer service, please complete the screen completely and accurately. All fields on this screen are required. Customer Information is confidential and is used only by the Street Division. On occasion it may be necessary to contact you for additional information to complete your service request. Once your service request is completed your email address is used to send a confirmation notice that will include your service request notification number.
When the customer information is completed, click Submit button to create service request.
Map Navigation Tool Bar
Map - Status Tools
The Map method's navigation bar consists of the following map navigational and notification search tools that will allow you to select, search and pin point map areas.
Viewing Tools
Pan: This function allows you to move the map without zooming in or out. Position your cursor anywhere on the map, click and hold the left mouse button, and drag the map to the area you would like to view.
Zoom Tools
Area-Selection Zoom In: This function allows you to zoom in closer to areas on the map. To use the zoom in tool, click on the tool and use your mouse to draw a "box" around the area you want to zoom to. Simply place the cursor at one corner of the area, click and hold the left mouse button, and drag the cursor to the other corner of your area and release.
Area-Selection Zoom Out: This function allows you to zoom out to view a larger map area. To use the zoom out tool, click on the tool and use your mouse to draw a "box" just like you would to zoom in. The smaller the box, the more you zoom out.
Direct Zoom In: Click on the "Zoom In" button to do a direct zoom in to a preset level.
Direct Zoom Out: Click on the "Zoom Out" button to do a direct zoom out to a preset level.
Direct Pan: Click on the "Pan" button first. Position your cursor anywhere on the map, click and hold the left mouse button, and drag the map to the area you would like to view.
Identify: Click on the "Identify" button when you are ready to select an item / inventory to which you are reporting the problem.
Note: The inventories such as street lights or traffic signals are displayed with different color coding and shapes for easy identification. They are displayed based on your problem type selection.
Select Street: Click on the "Select Street" button to select a street that relates to the problem. Click on this button and then click on the street in the map that you are reporting the problem.
Get Map: Click on the "Get Map" button to load the map based on your selection criteria of Address or Intersection/Community/Zipcode and Problem type. Also select "Street section" or "Intersection" if displayed beneath Problem type before clicking on "Get Map" button.
Legend for Symbols
Street section: This symbol identifies a Street section.
Intersection: This symbol identifies an Intersection.
Storm Drain: This symbol identifies a Storm Drain.
Street Light: This symbol identifies a Street Light.
Traffic Signal: This symbol identifies a Traffic Signal.
Service Request Status
View status of a Service Request
Get Current Status: If you have created and submitted a request for service through the Service Request website, you will soon receive an E-mail with a 12 digit number indicating the "Notification Number" that has been assigned to your request. This number, when entered in the "Get Current Status" field, will allow you to check on the status of your request. The results will be displayed with an explanation of what each status means.
